I'm surprised that you haven't updated the Futurama page in a long time, there's still about a dozen or so transformations that you didn't capture. Here's some that I remember (I might be forgetting a few):

"Wizzin'": Fry, Zoidberg and Bender are turned into toads.

"Three Hundred Big Boys": Farnsworth gets a treatment that makes him look younger. His girlfriend also gains a lot of weight.

"Teenage Mutant Leela's Hurdles": Everyone gets continually regressed down in age.

"Neutopia": Everyone gets their genders switched and back.

"The Prisoner of Benda": Everyone gets their minds and bodies swapped around.

"The Butterjunk Effect": Leela and Amy take super-steroids for muscle growth.

"Naturama": Everyone continually turns into animals.

"Leela and the Genestalk": Leela grows tentacles all over her body.

"Murder on the Planet Express": An alien shapeshifter disguises itself as the Planet Express crew.

There aren't really any true transformations on this show, but there are a couple of noteworthy episodes that could provide clips for this site:

"Stinkmeaner Strikes Back": In which Tom Dubois gets possessed by the ghost of Colonel Stinkmeaner. There are three different scenes involving Stinkmeaner taking control of Tom, all of which could easily be placed in the Mind Transfer category:

As this wasn't really a fantasy show, there wasn't much in the way of transformations. But here are some non-canon examples:

"Terrifying Tales of Recess": Cornchip Girl turns into a werewolf and back. There's two good scenes of this happening.

"Big Brother Chad": Vince has a nightmare about turning into a geek. Though I'm not sure if we have a category for this.

"Big Ol' Mikey": Mikey has a nightmare about turning into a giant. Though I'm not sure if they actually show him changing.
